Police have urged more victims of Simon Levy to come forward, as London's victims' commissioner stated a "litany of failures" allowed the convicted sex offender to murder two women and rape a third. Andrea Simon called for a review into the inadequate monitoring of Levy, a 40-year-old known to police as a sexual predator.

Levy was found guilty on Friday of murdering Carmenza Valencia-Trujillo, 53, and Sheryl Wilkins, 39, and raping another woman in London between September 2021 and September 2025. The Met Police confirmed it had investigated whether Levy was responsible for further unsolved incidents, including deaths, while British Transport Police (BTP) stated that women who recognised Levy as their attacker should report it.

The Metropolitan Police, BTP, and Crown Prosecution Service (CPS) have all acknowledged failures in how Levy was monitored during the period between his first conviction in September 2021 for sexual assault and his arrest for murder in September 2025. "There was a lack of restrictions that could have been placed on him and a failure to monitor him appropriately – so all of those things, I think, we need answers to now," Simon told BBC Breakfast, emphasizing the need for a multi-agency review.

Simon also suggested that public services must improve their ability to identify patterns of offending before attackers escalate their crimes. The Met Police indicated that detectives have pursued numerous lines of inquiry, including working with coroners, to determine Levy's potential involvement in similar, unresolved incidents, but no additional victims have been identified to date. However, the force reiterated its plea for anyone who believes they may have been a victim to contact the police.

Levy's offending history includes at least 10 attacks on women on public transport in London, where he targeted busy carriages. BTP noted that more women may recognise Levy and may not have yet reported their experiences. Levy was on bail and under police monitoring when he committed a violent rape in January 2025, before going on to murder Ms. Valencia-Trujillo in March and Ms. Wilkins five months later.

Earlier this year, a jury at Inner London Crown Court found Levy guilty of groping 10 women on the Tube and an 11th woman while he was in prison. Details of these assaults could only be reported after the conclusion of his murder trial. The prison offence occurred in April 2022, with the Tube assaults taking place between October 2023 and May 2025. He had previously been convicted of sexually assaulting two women in 2018, leading to a jail sentence in 2021.

The Independent Office of Police Conduct (IOPC) has confirmed that a police constable is under investigation for gross misconduct, and a detective sergeant is facing a misconduct investigation. BTP has also confirmed that two officers have been moved to different roles due to incompetence in handling investigations related to Levy.

Levy is scheduled to be sentenced on Wednesday. The full extent of his crimes and the systemic issues that allowed them to occur remain under investigation.
